I looked at it but unable to fill it out accurately for my situation!! Don't want to skew the results!! i.e I only treat chemically when going into area where ticks are issue. I only check for ticks when I have been walking in areas that dogs may get them (i.e no requirement to check after a beach walk!!  ) |

There was plenty of room for further comments. |
