Hi all, long time since I’ve been on here, hope everyone is well and having a good new year.
Just popping in as I could do with some advice, hoping someone might have experienced something similar and can give some insight(or reassurance!). Bit of a long one so I appreciate anyone sticking with the whole thing.
Ziggy was diagnosed with Hyperthyroidism about 7 years ago, she’s been on Thyronorm since and we’ve always mixed this in with small amount of wet food and had no problems. She was on a dose of 2ml twice a day. She is 16.
Over the Christmas period she stopped eating, began drinking & urinating more and started vomiting small amounts of white foam multiple times a day. I took her to the vet and they did some blood tests and other tests and found her T4(thyroid) level to be incredibly high. I’ve been told the normal T4 level is a range of 10-60, Ziggy’s came in it over 250, which is as high as the vets equipment is able to measure. Her calcium was also too high though not so dramatically. The vet kept her in on a drip over 3 days to rehydrate and flush out the calcium. We had her home over the weekend in the hope she would be able to self regulate a bit, Saturday was fine but sadly she stopped eating and threw up again on Sunday so it was back to the vet this morning for another anti nausea and appetite stimulant but she’s not really eating again today besides a tiny amount of boiled chicken.
The vet did X-rays and an ultrasound last week to check for cancer but is fairly confident that despite finding some enlarged lymph nodes in her stomach(apparently fairly common in older cats anyway) that it isn’t cancer.
I’m now giving her 5ml twice a day of the Thyronorm, straight into her mouth rather than mixed with food. We’ve also been given Famotidine (anti-acid) tablets to give once a day though I am struggling to get those down her.
If anyone has any experience with such high T4 levels that would be great, as I’m not sure our vet has experienced anything this high before. I’ve been worried sick all week and just want to help my poor girl feel better:(

Hi Weims. A Happy New Year to you too. Your poor cat Ziggy has certainly been through it, bless her. I don't know whether anyone on here has had experience with a cat with hyperthyroidism - possibly tonib with one of her past cats. I really hope that Ziggy continues to eat, even if it is only boiled chicken. Keep us informed.

Thank you lotsofcats, hope you and the cats are well. She's got more of an appetite today after another dose of anti nausea from the vet so fingers crossed it lasts!

Glad that Ziggy's appetite is better.
Sadly, I lost my oldest Maine coon, Kiba-Khan, at the end of November - he would have been 16 next month, which is a very good age for a Maine Coon. All my pets are getting older, my 2 youngest are 12 in May. Bramble (16) had cystitis last month and Kai Ruki's back leg keeps giving way (arthritis). Kobi the dog has lots wrong with him and he's 13 on Saturday.

Weims, good to see you back here but sorry to read the reason. Back in 2020 my cat, Roman, was diagnosed with Hyperthyroidism & then diabetes at age 16. Also Penny was diagnosed with thyroid issues back in 2015 aged 16, she already had kidney issues. I'll look back at the blood results I've got for both of them & see what I can find out that may help. How old is Ziggy now?

Sorry Weims , just realised you posted Ziggy is 16. I've looked back at the blood tests, can't find T4 values for Penny she was diagnosed as hyperthyroid as the vet found a goitre on her neck. She was a nightmare to pill & already had kidney disease. So I don't think she is one to compare with. Roman had T4 of 99 which came down to 69 after 3 weeks of felimazole but then he was diagnosed with diabetes as well & put on insulin. The diabetes was never fully stabilised over the next 2 years & as the most life threatening its treatment was prioritised. As the vet couldn't feel a goitre Roman's felimazole tablets were stopped as they could have been interfering with the insulin. 5 months later still no goitre. However another 2 months later his blood test showed high T4 so was started on Thyronorm & after another month his T4 was still high so the dosage was increased. Eventually 2 months later a goitre was found so the vet removed the left hand side of the thyroid & stopped the Thyronorm. But a month or so later & no impact on the diabetes instability he was restarted on the Thyronorm. He always had it direct from the syringe & loved it - always tried to get as much as he could from the syringe! Although he had the occasional vomiting, mainly fluid/foam but sometimes food, we never needed any anti-nausea or appetite enhancing meds. He was on the equivalent of 7 100g pouches a day to keep his weight on though out the 2 years of these 2 illnesses. Eventually it was all too much for him as it now included HCM and he was assisted to Rainbow Bridge in June 2022 aged 18. I don't think he ever went as high as Ziggy's T4. Has the vet suggested surgery on the thyroid or even I-131 treatment, although could be expensive. Sorry to hear about Kiba-Khan lotsofcats , I hope you are doing okay. Ahh bless them all getting older, it’s not easy to watch them slow down but hope they are all doing alright. Happy belated birthday to Kobi! Sorry it’s taken so long for me to reply tonib , been so busy. Thanks for all the info. I do worry that Ziggy has something else underlying wrong with her that’s hidden by the hyperthyroidism but I suppose I can’t speculate or worry about that! Ziggy has just finished her course of appetite stimulant and it’s definitely helped but now it’s stopped her appetite has dropped slightly again. I’ve joined a couple of Facebook groups for Hyperthyroid cats but haven’t posted anything - they don’t seem to be of much help as everyone just seems to complain about how Thyronorm has made their cats sicker and the starting dose of 2.5mg is too high(hence why I’m a bit apprehensive to post about her 5mg dose!!) and I-131 is the only way to go! Our vet hasn’t bought up surgery or I-131 which I assume is mostly due to her age. To be honest I wouldn’t consider the I-131 as it’s too expensive really and Ziggy’s such a needy girl the thought of not being able to cuddle her for the set period is unbearable. If you don’t need the extra Thyronorm that would be great, her insurance doesn’t cover her hyperthyroidism anymore as we’re at our limit and we’re getting through a 30ml bottle a month on her new quantity! I think my next step is to get her T4 retested and hope that if it’s stabilised we can reduce the dose and that’ll remove any lingering nausea and lack of appetite.

Roman was on .75ml (3.75mg) Thyronorm before the thyroid (1/2) removal. When he was put back on it he was on 1/2 that which didn't exactly work out with the syringe markings so made it 1.75mg. It was subsequently doubled back to 3.75mg (.75ml) Sorry about the confusion of units but Norbrook changed their syringe marking. Weims PM me your address & I'll send you the bottle I've still got. Also worth calculating what it would cost to get it online. I checked Animeddirect.co.uk, petdrugsonline.co.uk, vetuk.co.uk but the surprising cheapest (by quite a bit) was weldricks.co.uk a pharmacy chain in South Yorkshire (Doncaster way). You need to find out how much your vets would charge for a 6 month written prescription (for 1 bottle repeated as many times as needed to last 6 months) So if you use 1 bottle a month then 1 bottle with 5 repeats (total 6) add on any delivery charge from weldricks (don't forget you can order multiple bottles). I had a prescription for 6 bottles each 6 months & ordered 3 bottles every 3 months. You need to make sure the vet's prescription specifies the size of bottle as there is a 30ml bottle (which both you & I used) & a 100ml bottle (which is actually slightly cheaper) but I preferred to get the 30ml but if you use more maybe asking your vet to prescribe the 100ml might be better. In my case the vet charges £18 for a 6 month prescription which was for 6 bottles so £3 a bottle. Weldricks (at that time) charged £14.37 a bottle so £44.37 which is over there minimum for free delivery. So 14.37 + 3 = £17.37 compared to the vets charge, which I can't remember now but definitely dearer!! Out of interest I just checked their website & its now £14.89 still £4 a bottle cheaper than the other suppliers!!! Did your vets look for kidney disease in the blood tests, if hyperthyroidism is influencing the standard tests there is an SDMA test which picks up kidney disease earlier. Not eating, nausea, excess drinking & urination are symptoms/effects of kidney disease. However arthritis can also impact as bending to eat/drink from a bowl on the floor can be painful. Osiris was diagnosed with kidney disease in 2017 but just recently his values were well in the normal range. He has ben on new arthritis meds for a while now so his last blood test his T4 was tested - fine so this month (3 months on) he's going to have the normal in-house blood tests but also SDMA. Fingers crossed Zigy's T4 has improved.

Saw the vet on Tuesday and had Ziggy's blood fully retested(haematology, smear, chem-15, total T4) and so relieved to say that her T4 has come right down, in fact it was a little too low so we're trying her on 2.5mg Tyronorm again(down from 5mg), and her kidney results were good. She's still drinking & peeing a lot and I think only eating because we've restarted the appetite stimulant so I'm hoping that's just a side effect of the meds or the T4 now being too low? It seems strange that the symptoms haven't changed throughout but I feel as though if it were her kidneys and they were affected enough to be showing symptoms then they would show up in her bloods? Or do you think I should request the SDMA too? Thanks for the info regarding prescriptions, had no idea I could request a six month prescription, or that it would be cheaper from a human pharmacy! I used to use Vet UK for the 100ml which wasn't too costly in comparison to the vet but recently we've been using the 30ml from the vet as we've been going through it faster than I can keep up with and I run out of time to wait for postage if I order online! Glad Osiris is doing well and his tests looked clear last time, fingers crossed for good results this time around too It sounds like his kidney disease is managed quite well? Our vet made it sound like it'd be terminal if he'd found kidney issues in Ziggy but maybe that was with the mindset that her T4 may not have improved either.
